Evaluation / termination
 
Student evaluation of each work term / learning objectives
At the close of each work term, students must complete a Co-op / Internship Work Report (PDF file) and submit it to Career Services. The student will meet to discuss this with his/her co-op / internship advisor upon return to school, and a copy is sent to the student's academic department.
Employer evaluation of each work term
At the close of the work term the student is responsible for:
- asking her/his direct supervisor to complete a written evaluation.
- returning the evaluation to Career Services.
You may use a standard company performance evaluation or use the Employer's Evaluation of Co-op / Internship Student (PDF file).
We strongly recommend that you discuss the student's evaluation with her/him in person before the student leaves your work site.
A copy of your evaluation will be provided to the student's academic department.
Terminating a student:
If performance issues or other circumstances require termination of a student's employment before completion of all work terms, or during a work term, the employer is responsible for informing the student at once and for informing the student's co-op / intern advisor.
