Evaluation forms you must return to Career Services:
Return end-of-work-term evaluations (by you and your employer) to Career Services within eight weeks of the end of your work term.
 
Meeting with your advisor
Call (231-6241) or visit Career Services during office hours to schedule an appointment with your Co-op / Internship Program advisor to occur within the first eight weeks of the semester in which you return to school. You'll discuss your evaluation of the work term and learning objectives for your next work term.
 
Learning objectives
  If you have an upcoming work term, discuss with your advisor your proposed learning objectives for that term. You may complete your Learning Objectives form when you have sufficient information from your employer about your next work term.
You may use:
- PDF version (print and complete by hand) or
- MS Word version (complete using MS Word; print to obtain signatures)
 
Enrolling in CEP 4084 for upcoming work terms
If you will be working during an upcoming term, the Co-op / Internship Program administrator will automatically enroll you for CEP 4084 for the term(s) based on your Work / School Schedule, so it is essential that you follow the schedule.
