Calendar and deadlines
CEIP calendar of work and school term start and end dates and paperwork and meeting deadlines.
(This isn't really a form, but we thought you might find it helpful here.) 
Tells you the begin- and end-dates for upcoming semesters and work terms, and tell you deadlines for forms and meetings.
 
Learning Objectives Form: for start of work terms
For each work term, complete this form at the start of the term, working with your employer to complete appropriate sections as explained on the form. See the sample learning objectives (PDF file) to help you develop your individual objectives.

MS Word version
Complete using MS Word.
Save to your computer.
  (You'll need to refer to this later when you complete your work report!)
Print.
Sign and obtain your work supervisor's comments and signature.

Co-op / Internship Work Report Form: for end of work terms
  MS Word version - to complete using MS Word; save to your computer; send as attachment to e-mail to julie.richardson@vt.edu, by deadline on CEIP calendar.
  Complete this Work Report after each of your work terms.
On this report you evaluate the extent to which each of your learning objectives for the term was accomplished. If you work for two terms consecutively for your employer (e.g., spring through summer or summer through fall), you may submit one work report covering the two work terms.

Who sees it?  Your Career Services co-op / internship advisor reads it and a copy is placed in your co-op / internship file in Career Services. A copy is sent to your academic department. Otherwise it is confidential — other students don't read it and your employer is not given a copy. We may collect data from these reports, but it would not be reported by individual student name or identifiable information.

   
Employer / Supervisor Evaluation of Student: for end of work terms
  PDF version - to print and complete by hand
  MS Word version - to complete using MS Word; must then be printed to obtain handwritten signatures.
  Ask your supervisor to complete this and discuss it with you at the end of each of your work terms. You then submit it to Career Services within eight weeks of the end of your work term.

Final Report Guidelines
  When you complete all your co-op / internship work terms, see Final Report Guidelines to write your final report. We value your view of the experience and want other students to know about it. The guidelines tell who sees it and what it should contain and when it's due.
